صفحات 1164-1170مقدمهاریترومایسین دارویی است که با اثر بر گیرنده های موتیلین، باعث کاهش حجم کیسه ی صفرا می شود و به صورت تئوریک می تواند به پاره نشدن کیسه ی صفرا حین گرفتن با ابزار و یا برش از کبد کمک نماید. پژوهش حاضر با هدف بررسی تاثیر اریترومایسین بر کاهش حجم کیسه ی صفرا و به دنبال آن، کاهش میزان پارگی و درد پس از عمل انجام شد.روش هادر این مطالعه ی کارآزمایی بالینی، 130 نفر از زنان مبتلا به کله سیستیت مزمن به صورت تصادفی در دو گروه شاهد (63 نفر) و آزمایش (67 نفر) قرار گرفتند. به گروه آزمایش 250 میلی گرم اریترومایسین وریدی 30 دقیقه قبل از عمل جراحی تزریق شد و کاهش حجم کیسه ی صفرا به وسیله ی ابزار لاپاروسکوپی توسط یک جراح اندازه گیری گردید. بیمار جهت بررسی درد، 24 ساعت پس از عمل و یک هفته بعد مورد ارزیابی قرار گرفت.یافته هامیزان حجم کیسه ی صفرا در گروه آزمایش کاهش معنی داری را نشان داد (05/0 > P)، اما تغییر معنی داری در میزان پارگی آن مشاهده نشد (05/0 < P). درد پس از عمل نیز در بیماران گروه آزمایش کاهش معنی داری داشت (05/0 > P).نتیجه گیریاریترومایسین داروی موثری در کاهش حجم کیسه ی صفرا می باشد و این اثر به لحاظ عملی نیز مشاهده شد، اما تاثیر واضحی در کاهش میزان پارگی به لحاظ آماری نداشت. این دارو از طریق کاهش مدت زمان عمل جراحی و کاهش حجم کیسه ی صفرا، باعث کمتر شدن درد پس از عمل می شود. بنابراین، استفاده از اریترومایسین وریدی به این منظور توصیه می شود.کلیدواژگان: اریتروماتیسین، کیسه ی صفرا، کله سیستکتومی، لاپاراسکوپی

صفحات 1171-1177مقدمهانتقال ژن های مقاومت دارویی از طریق اینتگرون ها، شایع ترین راه گسترش مقاومت های آنتی بیوتیکی و به دنبال آن، پیدایش گونه های دارای مقاومت دارویی چندگانه (Multidrug resistance یا MDR) است. هدف از انجام مطالعه ی حاضر، تعیین فراوانی اینتگرون های کلاس I و II در میان ایزوله های اشرشیاکلی جدا شده از عفونت های دستگاه ادراری کودکان در کرمانشاه بود.روش هادر این پژوهش توصیفی- مقطعی، 89 ایزوله ی اشریشیاکلی جمع آوری شد. پس از تایید ایزوله ها با تست های اختصاصی بیوشیمیایی، حساسیت آنتی بیوتیکی آن ها با روش دیسک دیفیوژن مورد سنجش قرار گرفت. فراوانی اینتگرون های کلاس I و II با استفاده از پرایمرهای اختصاصی آن ها و روش Polymerase chain reaction (PCR) تعیین گردید.یافته هااز میان 89 نمونه ی مورد بررسی، 53 ایزوله (3/59 درصد) دارای MDR بود. بیشترین میزان مقاومت نسبت به آمپی سیلین (4/85 درصد) و کوتریموکسازول (5/68 درصد) و کمترین میزان مقاومت نسبت به ایمی پنم (4/12 درصد) و نیتروفورانتوئین (8/16 درصد) مشاهده شد. فراوانی اینتگرون های کلاس I و II به ترتیب 9/71 و 5/3 درصد بود. ارتباط معنی داری بین فراوانی اینتگرون ها و مقاومت به تتراسایکلین و جنتامایسین وجود داشت (050/0 > P).نتیجه گیریعلاوه بر فراوانی بالای ایزوله های دارای MDR، شیوع اینتگرون های کلاس I در ایزوله های جداسازی شده ی اشرشیاکلی بیشتر بود. در نتیجه، شناسایی فراوانی اینتگرون ها و ارتباط آن ها با الگوهای مقاومت دارویی در ایزوله های باکتریایی ضروری به نظر می رسد.کلیدواژگان: اینتگرون، مقاومت آنتی بیوتیکی، اشرشیاکلی

صفحات 1178-1184مقدمهسالیانه بیماران زیادی به دلیل نارسایی کلیوی نیاز به دیالیز پیدا می کنند. این بیماران، نیازمند مداخلات متعدد جهت باز نگهداشتن اکسس عروقی خود هستند که این امر، هزینه ی زیادی به سیستم درمانی تحمیل می کند. اثر داروهای ضد فشار خون بر بروز ترومبوز Arteriovenous fistula (AVF) روشن نیست. هدف از انجام مطالعه ی حاضر، بررسی تاثیر مصرف داروهای ضد فشار خون قبل از عمل AVF در بروز ترمبوز فیستولی، در بیماران نیازمند AVF بود.روش هااین مطالعه، یک پژوهش کارآزمایی بالینی بود که بر روی 60 بیمار مبتلا به نارسایی کلیه و کاندیدای انجام همودیالیز انجام شد. گروه مورد داروهای فشار خون خود را قبل از عمل دریافت کردند و گروه شاهد دارویی دریافت نکردند. فشار خون سیستول و دیاستول، فشار خون متوسط شریانی و ضربان قلب، قبل و حین عمل اندازه گیری شد. بلافاصله، 24 ساعت و یک هفته پس از عمل، بروز ترمبوز فیستول توسط همکار جراح ارزیابی شد.یافته هامیانگین سن شرکت کنندگان 95/14 ± 85/58 سال بود و 4/78 درصد از آن ها مرد بودند. میانگین فشار خون سیستول در گروه شاهد بالاتر بود، اما تنها قبل از عمل و تزریق بی حسی موضعی، این تفاوت معنی دار بود. میانگین فشار خون دیاستول و فشار خون متوسط شریانی قبل از عمل، در گروه شاهد به طور معنی داری بالاتر بود. نتایج نشان داد که بلافاصله (03/0 = P)، 24 ساعت (04/0 = P) و یک هفته بعد از عمل (02/0 = P)، بروز ترومبوز AVF در گروه شاهد به طور معنی داری کمتر از گروه مورد بود.نتیجه گیریمصرف داروهای ضد فشار خون قبل از عمل جراحی در بیماران مبتلا به پرفشاری خون کاندیدای تعبیه ی AVF، موجب افزایش احتمال وقوع ترومبوز در AVF می شود.کلیدواژگان: فیستول شریانی- وریدی، داروهای ضد فشار خون، نارسایی کلیه

صفحات 1185-1191مقدمهزایمان زودرس (Prematurity)، با درگیری های متعددی نظیر سپسیس، انتروکولیت نکروزان (Necrotizing enterocolitis یا NEC)، خونریزی داخل بطنی (Intraventricular hemorrhage یا IVH) و آپنه همراه است. در مطالعه ی حاضر، اثرات ویتامین E خوراکی بر بروز عوارض نارسی در نوزادان نارس بررسی گردید.روش هامطالعه ی حاضر، یک کارآزمایی بالینی بود که بر روی 76 نوزاد نارس در بیمارستان الزهرای (س) اصفهان در سال های 96-1394 انجام شد. نوزادان به طور تصادفی در دو گروه با سن و جنس همسان تقسیم شدند. برای 38 نوزاد در گروه مورد بررسی، 10 واحد ویتامین E از راه لوله ی اوروگاستریک به مدت 3 روز تجویز شد. گروه شاهد، تحت درمان با 1 سی سی آب مقطر از راه لوله ی اوروگاستریک قرار گرفتند. بیماران دو گروه از لحاظ سپسیس، NEC و IVH مورد ارزیابی قرار گرفتند.یافته هادر این کارآزمایی بالینی، 7/48 درصد شرکت کنندگان پسر بودند. میانگین سن نوزادان، 46/1 ± 49/28 هفته و متوسط وزن آنان 97/167 ± 02/1183 گرم بود. بروز سپسیس (77/0 = P)، NEC (53/0 = P)، پرفشاری خون (31/0 = P)، مرگ (45/0 = P) و IVH در روزهای چهارم (30/0 = P) و هفتم تولد (20/0 = P) در دو گروه معنی دار نبود.نتیجه گیریاستفاده ی خوراکی از ویتامین E از لحاظ آماری تاثیر قابل توجهی بر پیش گیری از سپسیس، NEC و خونریزی داخل بطنی نداشت. با توجه به زیست- دسترسی قابل کنترل تر مصرف خوراکی ویتامن E در مقایسه با استفاده ی داخل عضلانی و یا داخل وریدی و همچنین افزایش سپسیس و NEC در صورت استفاده از دزهای بالای ویتامین E در سایر مطالعات، مطالعات بیشتری با دزهای متفاوت خوراکی ویتامین E، پیشنهاد می گرددکلیدواژگان: زایمان زودرس، سپسیس، انتروکولیت نکروزان، خونریزی، پیش گیری

صفحات 1192-1196مقدمهاختلالات خواب در بیماران کلیوی تحت دیالیز شایع است. هدف از انجام این مطالعه، تعیین ارتباط کمبود ویتامین D با کیفیت خواب در این بیماران بود.روش هااین مطالعه ی مقطعی بر روی بیماران تحت دیالیز صفاقی انجام شد. اطلاعات دموگرافیک، اطلاعات مربوط به دیالیز و کیفیت خواب با استفاده از پرسش نامه ثبت و سطح سرمی ویتامین D اندازه گیری شد.یافته هااز تعداد 81 بیمار، 6/55 درصد کیفیت خواب ضعیف داشتند. فراوانی کمبود ویتامین D در بیماران با کیفیت خواب ضعیف 6/75 درصد و در بیماران با کیفیت خواب خوب 1/61 درصد بود (226/0 = P). بین سطح سرمی ویتامین D و نمره ی کیفیت خواب، رابطه ی معنی داری وجود داشت (273/0- = ضریب همبستگی، 014/0 = P).نتیجه گیریسطح سرمی ویتامین D با کیفیت خواب در بیماران تحت دیالیز صفاقی مرتبط است. بررسی اثر درمان با ویتامین D بر کیفیت خواب این بیماران توصیه می شود.کلیدواژگان: ویتامین D، دیالیز صفاقی، خواب

صفحات 1197-1202مقدمهاسکولیوز به انحنای جانبی در ستون فقرات گفته می شود. با توجه به این که عوارض بعد از عمل جراحی در این بیماران شایع است، مطالعه ی حاضر، با هدف بررسی میزان عوارض بعد از عمل جراحی فیوژن خلفی با ابزار در بیماران بالغ مبتلا به اسکولیوز با علت ناشناخته انجام شد.روش هادر این مطالعه ی مقطعی- توصیفی، میزان عوارض بعد از عمل جراحی در 93 بیمار دچار اسکولیوز با علت ناشناخته که تحت عمل جراحی فیوژن خلفی به همراه ابزار بودند، بررسی شد. نمونه گیری به روش آسان با مراجعه به پرونده های این بیماران انجام شد. همچنین، اسکولیوز بیماران بر اساس نوع بد شکلی به 4 گروه (طولی C شکل، توراسیک، توراکولومبار/لومبار و Double curve) تقسیم و عوارض آن مقایسه گردید.یافته هامشکلات تنفسی بعد از عمل جراحی، در 4 گروه بد شکلی اسکولیوز با هم اختلاف معنی داری داشت (009/0 = P). همچنین، میزان عوارض بعد از عمل 35 مورد بود که 20 مورد از آن مربوط به عوارض ریوی بعد از عمل جراحی بود.نتیجه گیریمیزان عوارض بعد از عمل جراحی در بیماران دچار اسکولیوز با علت ناشناخته که تحت عمل جراحی فیوژن خلفی با ابزار هستند، شایع می باشد. عوارض ریوی، شایع ترین عوارض بعد از عمل جراحی در این بیماران هستند.کلیدواژگان: اسکولیوز، عوارض بعد از عمل جراحی، فیوژن مهره ای

صفحات 1203-1209مقدمهلارنگوسکوپی فرایند شایعی در بیهوشی و بیماران مبتلا به مشکلات تنفسی می باشد که با بروز برخی عوارض در هنگام لوله گذاری یا پس از آن همراه است. پژوهش حاضر با هدف تعیین تاثیر استفاده از لیدوکائین موضعی روی تیغه ی لارنگوسکوپ، تراشه و کاف لوله ی تراشه بر تغییرات فشار خون و ضربان قلب حین لارنگوسکوپی و لوله گذاری تراشه و مقایسه ی آن با گروه شاهد انجام شد.روش هادر این مطالعه ی کارآزمایی بالینی، 275 بیمار کاندید لارنگوسکوپی به پنج گروه 55 نفره «اسپری لیدوکائین 10 درصد روی تیغه ی لارنگوسکوپ، اسپری لیدوکائین 10 درصد داخل تراشه، اسپری لیدوکائین 10 درصد روی کاف تراشه، اسپری لیدوکائین 10 درصد روی تیغه ی لارنگوسکوپ و داخل تراشه و روی کاف تراشه (هر سه مورد) و عدم مصرف لیدوکائین» تقسیم شدند. میزان فشار خون و ضربان قلب همه ی گروه ها در زمان پایه، قبل از انجام لارنگوسکوپی و دقایق 1، 3، 5 و 10 بعد از انجام لارنگوسکوپی بررسی گردید و مورد مقایسه قرار گرفت.یافته هاطی انجام مطالعه، گروه اسپری لیدوکائین 10 درصد روی تیغه ی لارنگوسکوپ و داخل تراشه و روی کاف تراشه، دارای پایین ترین و گروه شاهد دارای بالاترین ضربان قلب و فشار خون متوسط شریانی بودند. تغییرات ضربان قلب و فشار خون در پنج گروه تفاوت معنی داری داشت، اما تغییرات درصد اشباع اکسیژن در گروه ها معنی دار نبود.نتیجه گیریاستفاده از اسپری لیدوکائین 10 درصد روی تیغه ی لارنگوسکوپ و داخل تراشه و روی کاف تراشه، با کاهش معنی دار ضربان قلب و فشار خون در بیماران تحت عمل لوله گذاری همراه است. به نظر می رسد، این روش می تواند جهت کاهش عوارض ناشی از لارنگوسکوپی مورد استفاده قرار گیرد.کلیدواژگان: لارنگوسکوپی، لیدوکائین، لوله گذاری داخل تراشه

Pages 1164-1170BackgroundErythromycin is a medicine that can affect motilin receptors and thereby cause wrinkling of the gall bladder and, theoretically, is able to help in not rupturing the gallbladder while holding it on the grasper and/or in not dissecting the liver. Therefore, this research intended to study the effects of this drug on reducing the volume of the gallbladder, on the consequent reduction in the extent of perforation, and on reduction in complications resulting from perforation including infections and postsurgery pain.MethodsThere were 130 women with cholecystitis in this clinical trial. They were randomly divided into the control group (63 patients) and the case group (67 patients). The case group received 250 mg of intravenous erythromycin 30 minutes before surgery, and a surgeon used laparoscopic instruments to measure gallbladder distention. Complications during surgery were recorded and, 24 hours after the surgery and one week later, patients were evaluated to study postsurgery pain and infections.
Findings: The extent of distension in the case group was significantly reduced (P 0.05). Postsurgery pain decreased in the case group (PConclusionIt can be concluded that erythromycin was effective in reducing distension of the gallbladder, and this effect was observed in the surgeries that were performed. It had no statistically significant effect on the extent of perforation, but decreased postsurgery pain through reducing surgery duration and distention. Pages 1171-1177BackgroundOne of the most successful advances in bacteria is transmission of antibiotic resistance genes by integrons, which leads to the emergence of multiple drug resistant (MDR) species. The aim of this study was to determine the prevalence of class 1 and 2 integron among Escherichia coli (E. coli) isolated from children with urinary tract infection (UTI) in Imam Reza hospital, Kermanshah City, Iran, in 2016.MethodsIn this cross-sectional study, 89 Escherichia coli isolates were collected. After identification by biochemical tests, and evaluating antibiotic susceptibility tests using disk diffusion method, the frequency of class 1 and 2 integron were determined using specific primers and polymerase chain reaction (PCR) methods.
Findings: Of total of 89 studied samples, 53 (59.03%) isolates were multiple-drug resistant. The highest antibiotic resistance of isolates was to ampicillin (85.4%), and co-trimoxazole (68.5%), and the lowest was to imipenem (12.4%) and nitrofurantoin (16.8%). Frequency of class 1 and class 2 integron were 71.9% and 3.5%, respectively. There was significant relationship between the frequency of integrons and resistance to tetracycline and gentamicin (PConclusionThe results showed that, in addition to the high prevalence of multiple-drug resistant isolates, the frequency of class I integron was also high in Escherichia coli species. Therefore, identifying frequency of integrons and their relationship with drug resistance patterns in bacterial isolates seems to be necessary.Keywords: Integron, Antibiotic resistance, Escherichia coli -
Pages 1178-1184BackgroundThe number of patients with end-stage renal disease (ESRD) and the need for hemodialysis (HD) has increased and they needed repeated intervention to maintain access patency. Care of hemodialysis patients has considerable economic burden on health care system. The effects of antihypertensive medications on arteriovenous fistula (AVF) thrombosis are not clearly explained. This study aimed to evaluate the effects of continuing and discontinuing use of antihypertensive medications in peri-operative time on arteriovenous fistula thrombosis.MethodsThis was a clinical trial study on 60 patients with ESRD and hypertension, candidates for hemodialysis that divided into two groups. The case group received their antihypertensive drug before surgery and the control group did not receive. Systolic, diastolic, and mean arterial blood pressure and heart rate were evaluated before and during surgery; the incidence of thrombosis immediately, 24 hours, and one week after surgery was evaluated as well.
Findings: Patients with mean age of 58.85 ± 14.95 years were participated that 78.4% of them were men. The mean systolic blood pressure was higher in control group but this difference was just statistically significant before surgery and at the start of administering local anesthesia. Just before surgery, the mean diastolic blood pressure and mean arterial blood pressure were higher in control group significantly. The number of thrombosis in case group was higher significantly than control group immediately (P = 0.03), 24 hours (P = 0.04), and one week (P =0.02) after the surgery.ConclusionUsing antihypertensive medications in patients, candidates for arteriovenous fistula, increased the risk of arteriovenous fistula thrombosis.Keywords: Arteriovenous fistula, Antihypertensive drugs, End-stage renal disease -
Pages 1185-1191BackgroundPrematurity is accompanied with various complications including sepsis, necrotizing enterocolitis (NEC), intraventricular hemorrhage (IVH), and apnea. In current study, we assessed the effects of oral vitamin E on incidence of prematurity complications in premature neonates.MethodsThis was a clinical trial study on 76 neonates in Alzahra hospital, Isfahan, Iran, during 2015-2017. Neonates were randomly divided in two age- and sex-matched group. 38 neonates of case group were treated with 10 units of vitamin E through orogastric tube for three days. Control group were treated with distilled water through orogastric tube. Patients in two groups were evaluated in terms of sepsis, NEC and IVH.
Findings: In this study, 48.7% of patients were boys. Mean neonate's age and weight was 28.49 ± 1.46 weeks and 1183.02 ± 167.97 g, respectively. Incidence of sepsis (P = 0.77), NEC (P = 0.53), hypotension (P = 0.31), death (P = 0.45), and IVH in 4th (P = 0.30) and 7th (P = 0.20) day of birth was not significantly different between the two groups.ConclusionUsing oral vitamin E did not have statistically significant effect on prevention of sepsis, necrotizing enterocolitis, and intraventricular hemorrhage. Due to more controllable bioavailability of oral vitamin E in comparison to intramuscular and intravenous use of it, and increased risk of sepsis and necrotizing enterocolitis by using high doses of vitamin E in other reports, further studies with different doses of oral vitamin E are recommended.Keywords: Preterm labor, Sepsis, Necrotizing enterocolitis, Hemorrhage, Prevention -
Pages 1192-1196BackgroundSleep disorders are common in patients with kidney disease and on dialysis. The aim of this study was to assess the association between vitamin D deficiency and sleep quality in these patients.MethodsThis cross-sectional study was done on patients under peritoneal dialysis. Demographic data, dialysis information, and sleep quality were assessed using questionnaires and serum level of vitamin D was measured.
Findings: Out of 81 patients, 55.6% had poor sleep quality. Frequency of vitamin D deficiency was 75.6% in patients with poor sleep quality and 61.1% in those with good sleep quality (P = 0.226). There was significant correlation between vitamin D level and sleep quality score (correlation efficient = -0.273, P = 0.014).ConclusionSerum level of vitamin D is associated with sleep quality in patients under peritoneal dialysis. Evaluating the influence of treatment with vitamin D on sleep quality in these patients is recommended.Keywords: Vitamin D, Peritoneal dialysis, Sleep -
Pages 1197-1202BackgroundScoliosis is lateral curvature of the spine and postoperative complications are common in these patients. This study aimed to evaluate these complications in adult patients with idiopathic scoliosis.MethodsIn this cross-sectional study, postoperative complications of posterior fusion with instrumentation were evaluated in the 93 adult patients. Convenience sampling method was used and patients were allocated into 4 groups of scoliosis deformity types (Long c, thoracic, thoracolumbar/lumbar, and double curve). Complications were compared between the 4 groups.
Findings: There was a significant difference between the 4 groups regarding postoperative respiratory complications (P = 0.009). There were 35 cases with postoperative complications of them, 20 cases had respiratory complications.ConclusionPostoperative complications are common in the adult patients with idiopathic scoliosis undergoing posterior fusion with instrumentation. Pages 1203-1209BackgroundLaryngoscopy is a common procedure in general anesthesia; but some problems such as laryngospasm and tachycardia may occur during laryngoscopy. This study aimed to determine the effect of using topical lidocaine on blade and cuff of laryngoscope tube, and tracheal mucosa on changes of heart rate and blood pressure during laryngoscopy and intubation.MethodsIn a clinical trial study, 275 patients, candidates for laryngoscopy, were selected and randomly divided in 5 groups of 55. 4 groups received lidocaine spray 10% on laryngoscope blade, intratracheal, on the cuff of laryngoscope tube, on blade plus cuff and plus intratracheal, respectively, and the fifth group did not receive lidocaine. Heart rate and blood pressure were measured in base, before laryngoscopy, and 1, 3, 5, and 10 minutes after laryngoscopy and compared between the five groups.
Findings: Spraying lidocaine on endotracheal cuff and laryngoscope blade led to decrease of complications such as tachycardia, bradycardia, hypotension, and hypertension. So, heart rate and blood pressure were the highest in the control group and mean changes of heart rate and blood pressure were significantly different between the five groups (PConclusionSpraying lidocaine 10% of the blade and cuff of laryngoscope, and intratracheal led to decrease of heart rate and blood pressure and probably, these methods can be used for decrease of complications due to laryngoscopy.Keywords: Laryngoscopy, Lidocaine, Intratracheal intubation
